Inpatient Rehabilitation Team Explained

Our inpatient rehabilitation team members will work with you to make your stay a success. Your treatment team will be customized based on your specific needs. Our treatment team includes:

Medical Director and Staff Physician

  • Approves your admission based on medical criteria for rehab.
  • Coordinates your rehabilitation program.
  • Consults with medical specialists, as necessary.
  • Attends the weekly team conference to monitor your status and need for continued rehabilitation services.

Nurse Liaison

  • Performs a pre-admission assessment.
  • Presents your information to the medical director for admission approval.
  • Answers any questions you may have prior to admission.

Intake Coordinator

  • Identifies and verifies the financial resources for your rehabilitation stay.
  • Coordinates your admission to NRH.

Case Manager / Social Worker

  • Functions as your link with the rehabilitation staff.
  • Provides discharge planning.
  • Assists with your adjustment to your disability.
  • Coordinates your equipment needs.

Rehabilitation Nursing Team (RN, LPN, CNT)

  • Orients you to the rehabilitation program.
  • Provides 24-hour nursing care.
  • Provides instruction to you and your family.
  • Attends team conferences and keeps you and your family informed.
  • Participates with your case manager in planning your discharge and assists you and your family through the transition to home.

Physical Therapists and Certified Physical Therapy Assistants

  • Teach lower body strengthening and coordination.
  • Teach proper body mechanics, posture, energy conservation, relaxation, and safety during transfers and walking.
  • Teach safe stair climbing.
  • Fit knee braces and ankle supports.
  • Assesses home environment and recommends changes for safety and energy conservation.

Occupational Therapists and Certified Occupational Therapy Assistants

  • Teach upper body strengthening and coordination.
  • Teach cognitive and perceptual skills.
  • Teach self-care (dressing, bathing, grooming) and independent living skills.
  • Teach functional transfers (tub, shower, toilet, car).
  • Provide and train patients in the use of adaptive equipment (dressing aids, eating utensils, etc.)
  • Fabricate and fit splints.
  • Assess home furnishing arrangements and recommend changes to promote safety and conserve energy.

Speech/Language Pathologist

  • Provides communication training (understanding, talking, reading and writing).
  • Provides assessment and therapy for cognitive/linguistic problems.
  • Manages feeding and swallowing problems.
  • Screens hearing.

Registered Dietitian

  • Evaluates your nutritional needs.
  • Provides nutritional counseling.
  • Attends team conference and makes recommendations for dietary adjustments.

Pharmacist

  • Dispenses and monitors medications, evaluates drug interactions.
  • Attends team conference and makes recommendations for medication adjustments.
  • Performs medication reconciliation from pre-admission through discharge.
  • Reviews lab values for anticoagulant therapy.

Consulting Physicians

  • Available as needed to provide specialized medical oversight during rehabilitation.
