Referrals

Patients referred to NRH receive a pre-admission assessment by a nurse liaison to determine if they meet rehab medical criteria. Our medical director must give medical approval and many commercial insurance payers must pre-approve the admission. This can usually be accomplished within 24 hours or less. The direct referral phone line is (615) 650-2555. It is staffed Monday through Friday from 8 a.m. to 4:30 p.m.

Admission Criteria

To be considered for admission, patients must meet the following criteria:

  1. Be medically stable enough to actively participate in a vigorous therapy program (at least three hours of therapy per day five days per week).
  2. Need at least two types of therapies (physical, occupational and/or speech).
  3. Have clearly identified rehabilitation goals that can be achieved within a reasonable length of time.
  4. Have a well-defined discharge plan following completion of NRH's program.
  5. Require 24-hour rehabilitation nursing care.
  6. Agree to full participation in the rehabilitation program.

As long as you continue to show progress with the therapy program and meet the above criteria, you will be guided toward attaining the highest possible level of functioning. The rehab team will recommend discharge once you achieve maximum goals.
